# Nutrition and Progressive Multiple Sclerosis
In progressive MS, nutrition plays a role in supporting the nervous system and limiting inflammatory processes. This tab describes which nutritional approaches have been studied and what is currently known about them.
Ketogenic diet

A ketogenic diet is a nutritional pattern in which carbohydrate intake is severely restricted and the proportion of fats and proteins is relatively high. The body then uses fuels from fats (ketones) instead of glucose. This pattern is being investigated in progressive MS because it may influence the energy metabolism of nerve cells and inflammatory markers.
Recent research (2026) suggests that combinations of ketogenic nutrition with certain bioactive substances are being studied for their effects on mitochondrial function (the energy factories of cells). Studies in animal models suggest that this pattern can reduce inflammatory responses, but the strength and feasibility of this effect in people with progressive MS have not yet been established. It is an intensive nutritional pattern that requires close monitoring, as nutrients normally found in carbohydrates (vitamins, minerals, fiber) must be replaced.
Mediterranean dietary pattern

The Mediterranean nutritional pattern consists mainly of vegetables, fruit, nuts, fish, olive oil, and whole grain products, with limited red meat. This pattern is rich in antioxidants and substances that can reduce inflammation.
In MS, this pattern is being investigated because it has relatively much research on other inflammatory diseases and neurodegenerative conditions and because it is low in refined substances that can fuel inflammatory processes. Studies suggest that anti-inflammatory nutritional patterns may be beneficial for people with MS, although specific evidence for progressive forms is still limited. This pattern is practically applicable and provides many nutrients that support the nervous system.
Antioxidant-rich food and plant-based bioactive substances

This concerns foods and substances that naturally contain many antioxidants: green tea (EGCG), turmeric (curcumine), blackberries and other dark-colored berries, leafy and brassica vegetables. These substances can neutralize harmful oxygen radicals that play a role in MS-related nerve damage.
Research from 2026 shows that certain plant substances (such as curcumine and combinations with other polyphenols) can influence inflammatory markers and autoimmune responses in MS animal models. For people with progressive MS, true clinical studies are still scarce, but laboratory data and small studies suggest that a diet with many of these substances may potentially be beneficial. Risks are limited, although high doses of certain supplements can in rare cases cause interactions with medications.
Protein pattern and amino acids

Protein consists of building blocks (amino acids) that the nervous system needs for repair and maintenance. Recent research (2026) shows that certain amino acids (such as glutamine and threonine) occur in different concentrations in MS and may be associated with disease activity or severity.
Adequate protein intake is therefore relevant, but it has not yet been established what the optimal pattern is for each person. Because progressive MS can be accompanied by reduced mobility and muscle loss, protein-rich nutrition receives extra attention. However, evidence for a specific amino acid protocol is lacking.
Vitamin D

Vitamin D plays a role in immune regulation and has been extensively studied in MS. A meta-analysis from 2026 summarizes that vitamin status is associated with MS disease activity and progression, although it is not yet entirely clear whether vitamin D deficiency is a consequence or cause of MS.
For progressive MS, adequate vitamin D levels are considered supportive, especially because many people with MS have lower levels and because vitamin D regulates immune function. However, recommendations for appropriate levels are determined individually, depending on geographic location, skin pigmentation and any kidney function.
Vitamin B-complex

B-vitamins (B1, B2, B6, B12, folate) are important for nerve function and energy metabolism. In progressive MS, certain B-vitamins may be relevant through various mechanisms.
Some research points to possible beneficial effects of certain B-vitamins on nerve damage and energy mobilization in the brain and spinal cord, but evidence specific to progressive MS is limited. Deficiency in these vitamins occurs and can be prevented through diet or, if necessary, supplementation.
Alpha-lipoic acid and biotin

Alpha-lipoic acid and biotin are substances that play a role in cellular energy metabolism. Research from 2026 points to possible benefits in experimental neurodegeneration models.
In progressive MS this pattern is being investigated because nerve cells require a lot of energy and because both substances can affect immune regulation. Findings are still mainly from animal studies or based on mechanistic studies; clinical evidence in people with progressive MS is limited.
Fasting and periodic eating patterns

Periodic fasting (for example intermittent fasting or alternating days with limited intake) can influence inflammation markers and immune function. The pattern is being investigated because it may help regulate autoimmune reactions.
For MS these are very early studies, with mainly animal models or theoretical mechanisms. Caution is warranted, as fasting for people with progressive MS and possibly reduced mobility can also carry risks (malnutrition, muscle breakdown). This pattern should only be attempted under medical supervision.
Diets low in ultra-processed products and high in salt

Depending on the research, studies suggest that certain dietary components (high salt content, ultra-processed ingredients, trans fats) can amplify inflammatory reactions and T-cell reactions — precisely the processes that are problematic in MS.
A diet without or with fewer ultra-processed products and high salt is therefore considered supportive in MS, particularly because it can reduce inflammation markers. This is not MS-specific, but aligns with broader recommendations around prevention of chronic diseases.
