# Alternative treatment approaches in ALS

Many people with ALS seek additional options alongside standard treatments. Below we describe the main alternative approaches being investigated for potential benefit, and which of these have scientific support.

Electroacupuncture

Electroacupuncture is a variant of acupuncture in which weak electrical pulses pass through the needles. This has been investigated in animal studies with ALS mice.

The connection between your gut microbiome and brain disease is becoming increasingly clear, including for ALS. In theory, modifying your bacterial composition through diet or probiotics, for example, could play a role. However, this is still very experimental for ALS. There are no sufficient clinical studies demonstrating that this is effective. Research questions remain open about which bacteria are important, how they should change, and what the best way is to achieve this safely.

Stem cell treatment (uncontrolled private offerings)

On the internet and in certain private clinics worldwide, stem cell treatments are offered to ALS patients, often outside formal research settings.

These offerings are problematic for several reasons. First, scientific evidence that they are effective against ALS is lacking—most have never been properly studied in randomized trials in humans. Second, they carry risks: infections, tumours and other serious harm have been reported. Third, many of these clinics operate outside medical regulation, meaning oversight and safety are absent. Additionally, costs are usually very high. Also, hope in such treatments sometimes causes patients to delay more effective, well-researched options. If you come across information about this, discuss it thoroughly first with your own healthcare provider.

Mitochondrial interventions

The energy-producing structures in nerve cells (mitochondria) do not function well in ALS. Researchers are investigating whether these can be repaired or improved.

Laboratory work has established that mitochondrial dysfunction and mitochondrial DNA (mtDNA) damage contribute to nerve cell death in ALS. Various approaches are being investigated to address this. However, this is still in early research phases. For people with ALS, there are currently no available treatments based on these insights. Research in animals and cells continues, but it will take years before such an approach can be safely and effectively tested in patients.

Antioxidants and metabolic modulation

Some substances, particularly aimed at reducing oxidative damage and normalizing metabolic problems in nerve cells, are on the research agenda.

Laboratory studies and animal models have demonstrated that oxidative stress and disrupted energy metabolism are important in ALS nerve cell damage. Substances that act on these show certain beneficial effects in these established tests. For example, initial results from certain metabolic interventions are emerging. This means we cannot say whether such interventions actually help people.

Nutrition and supplementation

A healthy diet and certain nutritional supplements are tried by many patients.

There is no strong scientific evidence that specific nutritional supplements or dietary styles can slow or prevent ALS. A good eating pattern is important for overall health and can contribute to quality of life. Some ALS patients find that certain foods make them feel better or improve their digestion.
