Role of nutrition during treatment
Nutrition plays a supportive role during sarcoma treatment. The body needs extra energy and building blocks during chemotherapy and radiotherapy to repair healthy tissue and fight off infections. However, many patients experience eating problems due to treatment side effects: nausea, altered taste, throat irritation or decreased appetite.

Adequate nutrition helps maintain body weight and muscle mass better during intensive treatments, which is important for recovery and tolerance of therapies.
The goal is not weight gain, but preventing unintended weight loss and muscle wasting. What 'adequate' is depends on your individual situation, body composition and treatment phase. It is best to determine this in consultation with your doctor or dietitian who knows your situation.
Nutritional problems with chemotherapy
Chemotherapy can change taste, so food tastes different or metallic. Nausea, vomiting or diarrhea may also occur. With some types of chemotherapy, the mouth lining is damaged (mucositis), making eating and drinking painful.

Small, regular meals instead of three large meals help better against nausea than alternating between a full stomach and an empty stomach.
Food at room temperature or cold feels better to many patients than very hot food. Acidic food (citrus, yoghurt, vinegar) can partly compensate for taste changes and can also help against nausea. If it becomes difficult to get enough calories through food, nutritional drinks (ready-made or homemade) can be a way to still supplement energy and proteins.
Proteins and muscle strength
During and after sarcoma treatment, muscle tissue is broken down. This happens faster with weight loss, limited movement and certain medicines.

Adequate protein in the diet helps slow down or limit this muscle wasting.
Protein is found in meat, fish, eggs, dairy, legumes and nuts. How much protein you need varies per person and phase of treatment. A dietitian can calculate this with you based on your weight and situation.
Exercise helps keep the muscles that are still present stronger. Some hospitals offer 'prehabilitation': guided preparation for major surgery or intensive treatment, with attention to nutrition and light training. This can increase tolerance for treatment.
Vitamins, minerals and antioxidants
During cancer treatment, your body experiences a lot of stress. This raises questions about antioxidants (substances that 'catch' free radicals).

Some studies suggest that certain antioxidant supplements (especially vitamin E and beta-carotene in high doses) during chemotherapy may possibly reduce the effectiveness of certain medications, although this does not apply to all treatments.

High doses of antioxidant supplements do not prevent cancer recurrence and are not routinely recommended.
It is safer and more effective to get vitamins and minerals primarily from food (vegetables, fruit, whole grains) than through high-dose supplements. However, a deficiency in certain substances can develop due to the disease itself or the treatment. A blood test can reveal this. Vitamins such as B12, folate, iron and calcium are points that are regularly monitored.
If you consider supplements — also natural or herbal-based — always discuss this with your doctor or pharmacist, as some may interfere with chemotherapy or targeted agents.
Food safety and infection risk
Patients undergoing chemotherapy often have a low white blood cell count (immunosuppression) and therefore have an increased risk of foodborne infection.

Bacteria from raw or spoiled food can cause serious infections.
Hygiene is important: wash fruit and vegetables thoroughly, do not store food too long, avoid raw eggs and meat during periods of low immunity. Your doctor or hospital staff usually provide specific guidelines for each treatment phase.
Nutrition and radiotherapy
With radiotherapy to the abdominal area (retroperitoneal sarcomas), the intestines in the irradiated area can become inflamed, causing abdominal pain and diarrhea.

Nutrition adapted to these side effects (more fiber or less, depending on symptoms) helps improve comfort.
With radiation to the head and neck, swallowing can become difficult; in that case nutritional drinks or pureed food are needed.
After treatment
After active treatment ends, many eating problems gradually normalize. However, some may continue to have reduced appetite for a long time, or symptoms (such as taste changes) may persist for months.

Exercise and regular eating times promote recovery of appetite and body composition faster than without structure.

Special situations
In patients with retroperitoneal sarcomas (in the abdomen), permanent intestinal changes can occur after surgery that affect nutrition.

Adapting nutrition to these changes (less fat, more fluid, smaller portions) helps many patients.
Medications for nausea, diarrhea or other side effects can themselves affect food absorption; this is part of your treatment plan that is adjusted regularly.
