# Symptoms and Phases of Becker Muscular Dystrophy

Becker muscular dystrophy progresses very differently individually. Some people have noticeable muscle weakness as early as childhood, others only much later. This tab describes how the disease usually develops and which complaints can occur at different stages.

Early symptomatic stage (early symptomatic phase)

In this phase, the first obvious signs of muscle weakness begin to appear, usually in late childhood or early adulthood.

- Weakness in the pelvis and upper thighs (especially noticeable when climbing stairs, jumping or getting up from a chair)
- Trouble running or high-intensity activities
- Possible muscle contractions (where muscles shorten and restrict movement), especially around the ankles and hips
- Chubby appearance of calves (pseudo-hypertrophy), although it does not increase muscle strength

**Meaning for daily life:**
At this stage, most activities can still happen as usual, but they require more energy. Sport and physical activity are becoming more difficult. It is slow — not dramatic — but it is noticeable that certain efforts are going less.

The duration of this phase varies greatly from person to person. In population studies, the median age at which first symptoms occur is around 12—15 years, but this can vary significantly. Individuals vary considerably in how fast the disease progresses; some have little progression over years.

Intermediate stage

In this phase, muscle weakness gradually increases. Most patients are still able to walk but have more difficulty and need guidance or aids.

- Tendency to get tired more quickly

- Weakness in the arm and shoulder musculature can also occur

**Meaning for daily life:**
In this phase, many patients use assistive devices such as walkers or crutches for longer distances. Climbing stairs may be necessary with help. Daily activities are still ongoing, but require more adjustments. Work and school may still be possible, depending on the position.

**Details about this phase: **
This phase takes a very different amount of time. In population studies, the median age at which walking difficulties become more prominent is around 20—30 years, but this varies considerably. Some patients retain walking ability until a very advanced age, others lose it much earlier. This depends on many individual factors, including the specific mutation.

Late stage (late stage)

In the late stage, muscle weakness is extensive and people need help for mobility.

**Physical symptoms: **
- Severe weakness in legs; walking is no longer safe or possible without assistance
- Many patients use wheelchairs or spend a lot of time sitting
- Weakness in arms and torso increases
- Muscle contractions can cause more restriction
- Heart problems are becoming more likely (heart muscle weakness, arrhythmias); regular electrocardiogram (ECG) and echocardiogram become important
- Possible respiratory weakness during exercises; sleep apnea (respiratory arrest during sleep) may occur
- Swallowing difficulties may occur

**Meaning for daily life:**
Personal care, transport and many daily tasks require professional support. Working outside the home is usually no longer possible. Adapted home and aids (adapted toilet, shower, bed) are becoming essential. Family and caregivers play a major role.

**Details about this phase: **
This also applies to this stage: enormous individual variation. In large population studies, patients with Becker muscular dystrophy die on average in their 40s to 50s (median age depending on the study), but many patients reach considerably higher ages, particularly if heart weakness is well managed. These are population averages; individual prognosis depends on genetic factors, heart involvement and quality of care.

Heart involvement

Heart problems are an important aspect of Becker muscular dystrophy, although they are less severe than in Duchenne dystrophy.

**Symptoms:**
- In early stages: often no noticeable symptoms, but abnormalities may be visible on ultrasound or ECG
- Later: shortness of breath, especially with exertion
- Fatigue and exercise intolerance
- In some cases: heart palpitations (feeling of irregular beating)
- Swelling of ankles or legs (sign of heart decompensation)

**Meaning:**
Regular heart examination (ECG and echocardiogram) is important to detect heart problems early. Many patients can function well for many years with proper care.

- Muscle weakness is increasing faster than before
- You have more difficulty with familiar daily activities
- You experience unusual shortness of breath, especially with normal activities
- You feel chest pain or heart palpitations (irregular or rapid beating)
- You notice swelling of ankles or legs
- You are noticeably weaker in your arms or hands
- You develop swallowing difficulties

Your doctor monitors the disease regularly through examination and tests (blood, heart tracing, heart ultrasound) so that any problems are detected early.
