# Alternative Treatments for Sjögren's Syndrome

Traditional Chinese Herbal Mixtures

Multiple recently studied herbal combinations address underlying inflammatory mechanisms involved in Sjögren's. Examples include Jiajian Yiguan decoction, Maidong Dishao decoction, Qiju Dihuang Pill and Shaoteng decoction. These mixtures consist of plant-based ingredients, some of which (such as Astragalus, Salvia, Ophiopogon and Dendrobium) are also being studied separately.

ResearchediPositive results in clinical studies, not yet standard treatment

Laboratory studies suggest that certain herbal combinations can influence inflammatory pathways involved in the disease — for example through oxidative stress or immune responses. In animal models there is evidence for improvement of symptoms such as dry mouth and dry eyes. In patients, some studies have been conducted, including from China, reporting improvement of symptoms and possible protection against dental problems. Research into efficacy and safety in large patient groups, compared with placebo, is still ongoing. The ingredients and preparation methods can vary significantly between products.

Acupuncture and Laser Therapy

Acupuncture is applied in various forms in Sjögren's, including classical needle therapy and laser-based variants. The laser-based form (low-level laser therapy) has been specifically studied for dry mouth. There is also an ongoing randomised trial of acupuncture for more general symptom relief.

ResearchediPositive results in clinical studies, not yet standard treatment

Some clinical trials point to cautious improvements in dry mouth, possibly through local or systemic effects on salivary gland function. The research field is still small and results are mixed. For most forms of acupuncture, evidence on a larger scale is still lacking. The treatment appears safe when performed by trained professionals, but must not replace regular medical monitoring and standard treatments.

Mesenchymal Stem Cell Therapy

This is a growing research area in which stem cells (usually obtained from adipose or bone marrow tissue) are administered with the intention of reducing inflammation and repairing damaged tissues (for example salivary glands).

ExperimentaliOngoing in study setting, outcome still unknown

Studies are largely still in preclinical or early clinical phases. There is evidence that stem cells can modulate inflammatory responses and may potentially promote tissue repair, but documented safety and efficacy in larger patient groups is still lacking. Treatments outside recognised research centres cannot be recommended while evidence is incomplete.

Individual Herbal Constituents

Various plant components are being studied separately: total glucosides of paeony (Total glucosides of paeony), Dendrobium, American ginseng, and Achyranthes. These are usually given as an extract or powder.

ResearchediPositive results in clinical studies, not yet standard treatment

Laboratory data and animal experiments show that certain constituents can suppress inflammatory cells (for example Th17 cells and B cells) or activate antiviral and antioxidant pathways. In humans, effects on dry mouth, fatigue and inflammatory markers have been reported in small studies. Large randomised trials are lacking. The purity and dosage of extracts can vary.

Non-pharmacological Supportive Therapies

These include physical activity, psychological counselling, sleep optimisation and possibly nutrition — evaluated in studies such as FESSONA (aimed at fatigue).

ResearchediPositive results in clinical studies, not yet standard treatment

Structured counselling for fatigue shows improvement in initial studies. This type of approach is safe and usually supports regular treatment, rather than replacing it. Further data are being collected.

Nutritional Supplements and Special Diets

Supplements with antioxidants, omega fatty acids or micronutrients are regularly proposed. Special diets (for example anti-inflammatory eating patterns) are informally recommended.

UnproveniNo scientific evidence that it works

Studies specifically examining supplements or diets in Sjögren are scarce. Although some components theoretically seem worthwhile (for example to support inflammation regulation), direct evidence for effectiveness in this disease is lacking. Some supplements can interact with standard medications, so caution is warranted.

Homeopathic preparations

Highly diluted substances prepared according to homeopathic principles.

Advised againstiProven ineffective or harmful, or dangerous in combination with your treatment

There is no scientific evidence that homeopathic preparations are effective for Sjögren. A patient who chose exclusively homeopathy instead of recognized treatment would remain without necessary effect and without adequate control of inflammation and symptom relief. Some homeopathic products can unexpectedly cause allergic reactions.

Products based on soy or isoflavonoids

These are sometimes suggested because of theoretical hormone-modulating effects.

UnproveniNo scientific evidence that it works

Specific studies in Sjögren are lacking. There is no reason to assume these products improve the underlying immune pathology.

Integration with regular treatment

Many patients consider alternative approaches alongside their standard treatment. It is important to know:

- **Interactions**: Herbal extracts and some supplements can interfere with medications (anticoagulants, immunosuppressants, anti-inflammatory agents).
- **Not a replacement**: Alternative approaches have so far not been effective enough for standalone treatment of the underlying autoimmune disease.
- **Symptom relief**: Some approaches (acupuncture, certain herbs) focus more on symptom management than on the disease itself.
- **Cost and availability**: Many studied Chinese herbal mixtures and stem cell treatments are expensive and not standard reimbursed.
