# Alternative treatments for primary lateral sclerosis
Primary lateral sclerosis (PLS) is a rare neurodegenerative disorder for which, to date, no medication is available that stops or reverses the disease. This has led many patients and their loved ones to turn to complementary or alternative approaches. On this page we describe which treatments have been studied and what the evidence says about them. Important: none of the methods mentioned here replaces regular medical care and supervision.
Acupuncture and Electroacupuncture
Acupuncture is a traditional Chinese treatment method in which thin needles are placed at specific points on the skin. Electroacupuncture adds electrical stimulation to these needles.
In recent years, various studies have been conducted on electroacupuncture in motor neuron diseases, particularly in ALS (amyotrophic lateral sclerosis). Animal studies with ALS mice have provided evidence that electroacupuncture at specific points (such as "Zusanli") may influence inflammatory responses in the nervous system. Some studies suggest that certain immune cells (microglia) become less active, which theoretically could reduce harmful inflammation.

However, the studies so far have been conducted almost exclusively in laboratory animal models. There is no well-conducted clinical study in PLS patients demonstrating that acupuncture slows the disease itself or modifies symptoms. A few small studies in ALS patients report cautious findings, but these are not large enough to draw firm conclusions. Acupuncture appears safe when performed by trained practitioners, but it should not be used as a replacement for contact with your doctor.
Microbiome-targeted approaches
The microbiome consists of billions of bacteria and other microorganisms in your gut. Recent research suggests that this microbiome may play a role in motor neuron diseases such as ALS. The idea is that changes in gut bacteria contribute to inflammation and harmful processes in the nervous system.
Microbiome-targeted approaches consist of modifying diet (more fiber, certain food groups), possibly using probiotics (live bacteria), and in some cases fecal microbiota transplantation (transferring healthy gut flora).
ExperimentaliOngoing in study setting, outcome still unknown
This area of research is still in an early stage. There is evidence that the microbiome is indeed involved in ALS, but this has not yet translated into proven treatments for PLS. There are no randomized clinical trials demonstrating that specific microbiome interventions overcome the disease or improve symptoms. Nevertheless, some research groups are investigating this further, particularly because it is low-threshold and relatively safe.
Nutritional supplements and natural substances
Many patients try dietary supplements or natural remedies, such as vitamins, minerals, herbal extracts and special amino acids. Some of these are promoted with the argument that they reduce inflammation or protect nerve cells.
An example is 6-shogaol, a substance from ginger plants, which is said to have neuroprotective effects in laboratory studies. Other frequently tried supplements are vitamin E, CoQ10, acetyl-L-carnitine and glutamine.
UnproveniNo scientific evidence that it works
Although some of these substances show interesting effects in cell cultures or animal models, evidence from clinical studies in PLS patients is lacking. Many supplements have not been well tested for safety in combination with regular medications. Some may interfere with certain medicines or blood thinners. It is important to always discuss this with your healthcare provider before starting to use supplements.
Mitochondrial therapies
Mitochondria are the 'energy factories' of your cells. Damaged mitochondria can contribute to cell death in neurodegenerative diseases. Recent research focuses on ways to repair mitochondrial DNA (mtDNA) or support the energy production of cells.
This is still very rigorous research work: scientists are testing in the laboratory whether it is possible to address defects in mitochondrial DNA or reverse mitochondrial dysfunction.
ExperimentaliOngoing in study setting, outcome still unknown
These approaches largely exist as a research direction, not as an available treatment. There are no clinical trials in PLS patients. The technology to safely and specifically repair mitochondrial DNA is still in an early stage of development.
Training programs and physical therapy
Physical exercise and targeted exercise programs are frequently offered to PLS patients. Some are aimed at maintaining muscle strength (resistance training), others at mobility and relaxation (stretching, yoga).

Physical therapy and moderate exercise have been investigated in motor neuron diseases with mixed results. Most guidelines recommend that patients remain active and maintain movement adapted to their abilities, because immobility can cause additional problems (muscle contractures, blood stasis). However, there is no evidence that intensive training programs overcome the underlying disease. Targeted advice from a physiotherapist experienced with neurological conditions can help perform adapted exercises safely.
Homeopathy
Homeopathy is based on the principle of 'like cures like': substances that cause symptoms would induce healing if administered in high dilution. This position is at odds with current pharmacology and neurology.

There is no scientific evidence that homeopathic remedies are an effective treatment for PLS or any other progressive neurological condition. Moreover, relying on homeopathy can lead patients to delay or not follow regular medical care. Many homeopathic 'remedies' are diluted to the point where no active substance remains.
Stem Cell Therapy
Stem cell therapy is based on the idea that stem cells can form new nerve cells or replace damaged cells. This is sometimes offered in private clinics, often abroad.

Research with stem cells takes place in laboratory settings and in preclinical trials, especially for ALS. For PLS, this line of research is less advanced. Many of the stem cell treatments now commercially offered have not been rigorously tested and are not scientifically validated. Safety and effectiveness are unclear. Patients considering undergoing stem cell therapy are strongly advised to discuss this extensively with their own physician first and to be cautious about clinic marketing and unfounded promises.
Integrative approaches (nutrition, psychosocial care, sleep)
Some practitioners advocate for 'integrative' approaches that combine conventional medicine with attention to nutrition, stress, sleep quality, psychological well-being, and social support.
ResearchediPositive results in clinical studies, not yet standard treatment
This is not a specific 'alternative' treatment, but rather a mindset of care provision. Some aspects (good sleep, managing anxiety, good nutrition) are important for health in themselves. There is consensus that psychosocial support and symptom management (for example, spasticity, fatigue) should be part of care for PLS patients. However, these do not replace disease-modifying therapies.
