# Alternative treatments for liver cancer
Traditional Chinese herbal combinations
Various Chinese herbal preparations are being investigated for possible effectiveness against liver cancer or against complications of cirrhosis that increase the risk of liver cancer. Examples include Jianpi Huayu decoctions (aimed at breaking down scar tissue in the liver) and Fuzheng Huayu tablets (aimed at preventing further deterioration in advanced liver damage).
ResearchediPositive results in clinical studies, not yet standard treatment
These formulas have been studied primarily in laboratory studies and smaller clinical trials, especially in Asia. They appear to possibly act on processes involved in liver inflammation and scarring, but there have been no large international studies confirming their effectiveness against the cancer itself. Patients sometimes take such preparations alongside regular treatment, but scientific evidence for benefit is still lacking.
Natural polysaccharides and plant extracts
Polysaccharides (sugar chains) from certain mushrooms, lichens and plants are being investigated in laboratories for possible effects on the immune system and cancer cells. Some plant extracts (for example from Angelica sinensis, a traditional Chinese plant) also show certain anti-cancer properties in cell studies.
ExperimentaliOngoing in study setting, outcome still unknown
These substances are still largely in the stage of laboratory research and computer simulation. To date, very few clinical trials in humans with liver cancer have been conducted. Most data comes from in-vitro research (in a petri dish) and animal studies. It is unknown whether these effects translate to effectiveness in patients.
Astragaloside IV and related immunomodulators
Astragaloside IV is a substance from Astragalus roots (a traditional medicine in Asian medicine). Laboratory research describes protective effects on liver function and possible stimulation of certain immune cells.
ResearchediPositive results in clinical studies, not yet standard treatment
Effects have been observed in cell cultures and animal models, but well-conducted clinical trials in human patients with liver cancer are very limited. It remains unclear whether and to what extent this plant would be effective as a supplement to standard treatment.
Quercetin and similar substances from plants
Quercetin is a common antioxidant found in vegetables, fruits and tea. In theory, it might have some protective action against cancer, but data for it comes mainly from computational or cell studies.
UnproveniNo scientific evidence that it works
There is no clinical evidence that quercetin or related plant antioxidants prevent or cure liver cancer. Much research on it is still purely theoretical. It can be part of a healthy dietary pattern, but should not be considered a cancer treatment.
Dietary particulars ('detox', strict protein restriction)
Some patients try alternative dietary regimens, ranging from 'detoxification cures' to severe protein restriction, hoping to prevent liver cancer. These are usually not designed based on scientific research into liver cancer.
Advised againstiProven ineffective or harmful, or dangerous in combination with your treatment
Strict dietary restrictions without guidance from a nutritionist can lead to malnutrition, which weakens immunity and healing capacity. This is especially dangerous in patients with advanced liver disease, who usually already have nutritional problems. 'Detox cures' have not been proven effective against cancer. Good nutrition under the guidance of a nutrition specialist is important, but it is not an alternative to regular treatment.
Imipramine in combination with sorafenib
This topic appears in recent research: the antidepressant imipramine could potentially enhance the effectiveness of sorafenib (a standard drug for liver cancer) in laboratory studies, especially when tumors become resistant to sorafenib alone.
ExperimentaliOngoing in study setting, outcome still unknown
This is a very young research topic. So far it has only been studied in cell cultures and animal models. Clinical application is still years away. Imipramine is an old medicine with known side effects, and combination with sorafenib resistance is not yet an established treatment.
Improvement of immune function ('immunotherapy' outside clinical trials)
Some alternatives focus on overall immune enhancement through supplements, special diets, or 'wellness' programs. This differs from regular immunotherapies (such as checkpoint inhibitors) that are applied in clinical settings.
UnproveniNo scientific evidence that it works
Although a well-functioning immune system is beneficial, there is no evidence that supplements or 'detox' programs replace or improve immunotherapy against liver cancer. Regular immunotherapy (for example nivolumab or atezolizumab) has clinically proven efficacy and is given under close medical supervision.
Acupuncture and manual therapies
Acupuncture and massage are sometimes used as adjuncts, especially for symptom management (nausea, fatigue, pain).
ResearchediPositive results in clinical studies, not yet standard treatment
For certain side effects (such as chemotherapy-related nausea) there is some scientific evidence for acupuncture. However, it does not inhibit tumor growth itself. It can contribute to comfort and well-being, provided it does not delay regular treatment and is done under the supervision of physicians who are informed.
Warning regarding simultaneous use
If a patient considers alternative treatments alongside regular cancer treatment (surgery, chemotherapy, targeted agents, immunotherapy), it is essential to discuss this with the oncologist. Some herbal preparations can interfere with chemotherapy or immunological agents, or can place extra strain on the liver. This is especially important in patients with advanced cirrhosis.
