# Treatment options for vascular dementia
The treatment of vascular dementia focuses on two goals: slowing cognitive decline and preventing further brain damage from strokes and vascular incidents. This is done primarily by controlling risk factors (such as high blood pressure and diabetes) and through supportive measures for daily functioning. There is no medication that can completely reverse vascular dementia, but prevention and early intervention can slow its progression.
Treatment of risk factors

Controlling elevated blood pressure is one of the most important measures to prevent future strokes and further cognitive decline. This is usually achieved with antihypertensive medications (blood pressure-lowering drugs), of which there are several classes: ACE inhibitors, AT1 antagonists, calcium channel blockers and diuretics. The choice depends on personal factors and any additional conditions.
Research shows that maintaining a reasonable target level (usually around 130–140 mmHg systolic) is important; blood pressure that drops too far can also pose risks, especially in older adults. Many patients do not notice a direct improvement in memory, but the aim is to prevent new damage.
Common side effects of antihypertensive drugs are fatigue, dizziness, cough (with ACE inhibitors) and disturbed electrolyte levels. These are monitored regularly.

Diabetes significantly increases the risk of blood vessel and brain abnormalities. Good blood sugar control (usually via insulin, oral medications or lifestyle) helps limit vascular damage. Recent research (2026) investigated whether certain diabetes medications (GLP-1 receptor agonists versus long-acting insulin) offer more protection against dementia, a topic that continues to be explored.
Side effects of diabetes medications vary widely: some can cause hypoglycemia (low blood sugar), others gastrointestinal complaints. This is discussed regularly with the treating physician.

Statins and other cholesterol-lowering medications (such as ezetimibe or PCSK9 inhibitors) are used when LDL cholesterol is elevated or there is already cardiovascular disease. They work by slowing arterial calcification (atherosclerosis) and improving the stability of existing plaques.
Many patients tolerate statins well. Muscle complaints and liver enzyme changes can occur and are checked periodically.

Healthy weight and diet that is good for blood vessels (such as the Mediterranean pattern) help improve blood pressure, cholesterol and diabetes control. Recent studies investigate the role of specific nutrients: vitamin K (in vegetables and nuts) and nuts themselves appear favourable from observational data, although direct dementia prevention has not yet been formally demonstrated.
These are not medicines but lifestyle adjustments; practical guidance takes place through dietitians and general practitioners.
Prevention of new brain infarcts

For patients with vascular dementia or related strokes, anticoagulation therapy is usually considered:
- **Acetylsalicylic acid (aspirin)**: a platelet inhibitor, mainly used to prevent new ischaemic infarction.
- **Clopidogrel and other P2Y12 inhibitors**: alternative or addition, especially after stents or certain strokes.
- **Oral anticoagulants (such as apixaban, rivaroxaban)**: especially necessary if there is atrial fibrillation or other clotting risks.
Overall side effects are increased risk of bleeding (including gastrointestinal bleeding) and sometimes skin reactions. This risk is weighed against the benefit of vascular protection.
Symptomatic treatment

Ginkgo biloba is sometimes used with the idea that it improves blood flow, but the evidence is weak. Medicines such as donepezil (a cholinesterase inhibitor) can offer modest benefit in some cases, but vascular dementia responds to this less well than Alzheimer's. These medicines work by inhibiting the breakdown of the signalling substance acetylcholine.
Side effects include nausea, diarrhoea and slow heart rate (bradycardia).

Depression is frequent in vascular dementia and can be both a consequence and an exacerbation of it. Selective serotonin reuptake inhibitors (SSRIs) are first-line and can help improve both mood and some cognitive functions. Recent systematic reviews (2026) emphasise the importance of early recognition and treatment of depression.
Side effects include apathy, sexual dysfunction, and hyponatraemia (low sodium levels). These are monitored.

Agitation, sleep disorders and inappropriate behaviour can occur. Non-pharmacological approaches are used first (daily structure, safe environment, support). When necessary, atypical antipsychotics (such as risperidone) or sedating agents (for example melatonin or certain antidepressants) are introduced cautiously, as older adults and dementia patients are sensitive to side effects.
Examination and monitoring

MRI examination (including diffusion tensor imaging) is used to determine the extent and location of vascular damage and to monitor disease progression. Social isolation is a risk factor for dementia decline. Regular contact, activities and community involvement contribute to cognitive and emotional health.
